ARMED FORCES VETERANS

Our armed forces champions for the practice are Jane Edwards and Dr F Parsons

Our Armed Forces champion will offer advice and support and will be able to signpost our Military community to the services they may require.

Please let the practice know if you have served in the Armed Forces as a Regular Reservist or an Adult Volunteer Cadet. We have a pack that you can collect from our reception.

The practice works with Sunderland CCG Armed Forces Leads and Veterans in Crisis Sunderland to ensure those who have served or are currently serving in the Military have access to information across the City on services and support that is available.

THE GREEN IMPACT SCHEME

This practice is delighted to be signed up to the Green Impact for Health Toolkit and is actively taking steps to reduce our carbon footprint. The climate emergency is also a health emergency, and health care in the UK contributes 5.4% of the UK’s carbon emissions.

We need to enhance the health of our current population without compromising the health of future generations, and this means operating within the sustainable boundaries of the planet. Fortunately most of the solutions for the planet also make our health better – for example eating a mainly plant-based diet, engaging in active travel and exercise in nature, avoiding smoking, reducing pollution etc all enhance our health and reduce our need for medications, with further benefits to the NHS, your health and the carbon footprint of healthcare.

THE NATIONAL DIABETES AUDIT (NDA)

The National Diabetes Audit (NDA)

This practice is taking part in the National Diabetes Audit (NDA)

The NDA collects information about diabetes care from GP practices and hospitals and is used to help the NHS to improve care for patients with diabetes.

It is managed by the Health and Social Care Information Centre (HSCIC), working with Diabetes UK and Public Health England.

The information that the audit collects is controlled by law and strict rules of confidentiality.

The NDA only uses information about your diabetes care that is already recorded, for  example, type of diabetes, latest blood pressure result and results of HbA1c, or eye screening tests. The NDA is not a research project. No extra blood tests, appointments or scans are needed.

 

You can choose not to take part if you have any concerns

If you do not want your information to be used, please inform the receptionist, your GP or nurse. They will make sure that this is noted on your medical records, so your information is not included. This will not affect your care in any way.
